New Shared Care Agreement requests

Please be aware that in accordance with the recommendation of several professional bodies, TBMG will not be accepting any new shared care agreement requests from private ADHD providers.  
 
We will continue to offer ADHD shared care prescribing in partnership with NHS specialists, as long as appropriate monitoring and care is provided.

Private Work Fees

There are certain services provided by our doctors which are not covered by the NHS for which you may be asked to pay a fee. This includes private sickness certificates, medical examinations and reports. For most non-NHS services advance payment will be required when you make the request.

A full list of Non-NHS services and charges is displayed in reception at both our surgeries and can be found below:

  • The following services are NOT provided free under the NHS and require payment.
  • Payment must be by cash or cheque (with guarantee card).
  • Where more than one form is completed, the charge stated will apply per form.

Please note - the practice will NOT undertake private reports or examinations for any persons not registered with the practice.

Certificates:

  • Private Sick Note: £24.50
  • Private Prescription: £24.50
  • Freedom from infection certificate (Poultry Factory etc): £68.50 Plus cost of tests if needed
  • Holiday Cancellation: £31.00 for 1st page plus £21.00 for each subsequent page
  • Fitness to Travel Certificate: £33.50
  • Police Force Applications: £52.00
  • OFSTED Report: (give to GP with notes, patient to collect & pay) £73.50
  • Diabetic Patients - Letter to fly: £24.50

Reports:

  • Sickness Report: £31.00 for 1st page plus £21.00 for each subsequent page
  • BUPA/PPP Claim Cerification £61.00
  • Seat Belt Exemption Certificate: £130.00
  • Reports for housing etc. at patients request: £61.00 - £73.00
  • Camp America (completed by GP Partners only): £30.00 for 1st page plus £21.00 for each subsequent page
  • Gym/Health Clubs/ Sports etc: It is the Practice policy not to complete certificates of fitness for Gyms & Health Clubs, weight loss programmes, sporting activities, flying, gliding, diving etc. Healthy Start vouchers should be completed by Health Visitors, not GPs.

  • Full Physical exam & report: £181.00
  • HGV Medical: £169.00
  • Vitalograph recording & report: £56.00
  • ECG recording & report: £116.00
  • Audiogram: £74.00
  • Venepuncture: £46.00 (PLUS charges for blood tests dependent on laboratory fees per test)

 

Other Services:

  • ACWY Vax for travel (Vaccination + certificate) £130.00
  • Anti-Malarial Prescription: £24.00
  • GDPR Data acess request: Dependent on data requested
  • Army Forms: Get in touch.
  • Medical reports for Employers etc: £374.00 per hour + £16.50 admin fee
  • Insurance companies: GPRs £205.00
  • Targeted reports: £173.00
  • Subject access records: First request free of charge, further requests on an individual basis in accordance with GDPR
  • DNA Testing:
    • Blood: £91.00 per patient
    • Saliva £60.50 per patient
  • Shotgun reports: (requested by police) £76.00
