NHS SUMMARY CARE RECORDS Following the local implementation of the Summary Care Record (SCR) for residents of Teesside in 2010, Summary Care Records have now been created for all patients registered with the practice (excluding those patients who had expressed a wish to opt out). The SCR is an electronic record of patients' key medical information derived initially from their GP records which can be shared between health care staff involved in that patient's care. Further information on the project is available from the following sites: What do you need to do to create an NHS Summary Care Record? Nothing. If you do not complete an 'opt out' form, it will be assumed you are happy to have a Summary Care Record and the process of creating a record will begin. What do you need to do if you decide not to have a Summary Care Record? You should complete an opt out form and send it to the surgery. A code will be placed on your computer record at the surgery which will ensure that you are not included in the SCR program.
